You at least need Hide modifiers for equipment and certain actions. Why is option 3 the worst? Their used to be special rules for high-powered campaigns, and other special rules for low-powered campaigns. There were whole sourcebooks for playing in "realistic," low-magic campaigns. Not saying I liked them, but it's not that much more work to make a good so...

I like the Oberoni approach. But sometimes that leads to arguments, which isn't fun. It's never fun to have to explain to somebody why their pet strategy (say, casting Spiritual Weapon every round) just sucks.
