Hit roll, effect roll and some kind of defence roll (soak or whatever) works fairly well IME. A friends heavily house-ruled nWoD campaign uses this, so did an Alternity game I played. Shadowrun's counting successes (& WoD equivalents) let you combine hit and effect roll & still seem to work,...
