The easiest system I've seen so far is the existing LA system combined with the LA reduction in UA. Nobody ever plays LA +2 in our games, but there's people who play LA +1 now. All things begin equal, it amounts to a flat 4,000 XP penalty.

That's the real problem. The only time minutes ever make sense is at early levels (before 5th). Combat never lasts more than 2 minutes. If it does, something strange is going on.

That's why you just take remedy moderate wounds from Magic of Faerun. That actually grants normal fast healing, not the "remember to track when you take your damage" stupidity from MotW.

Of course, since MoF is older, and the spell duration is different....

That might work (I'm not convinced Elusive Target makes the creature FF wrt you). In any case, it only works once. After that, they simply don't flank you, and don't take AoO against you.

The other drawback is that the object has to be hidden in the first place. You're not going to be able to hide a longspear.

And assuming you can convert all threats to crits. Things like Power Crit do weird things to the numbers.
