Be accurate. Violent rapes are rarely depicted in the detail that would call for an X rating, and women enjoying consensual sex are easily PG-13 if the details are suggested rather than shown.

It rather depends on the game system. In D&D and games it inspired, equipment is a fundamental aspect of character development, and being able to make things can easily destabilize things like 'balance', so a whole lot of work is required to ensure that a system doesn't lead to clearly superior ...
